For those who already have an Islamic bank account, just fill out the direct debit biller registration form.
For those who do not have an Islamic bank account, you are asked to fill out the form below

2) Borang Pembukaan Bank Islam (CIF 01, CIF 02, TANDATANGAN SPESIMEN)
